The Medicare GLP-1 Bridge Program Launch: What You Need to Know

July 1, 2026 marked the official launch of the Medicare GLP-1 Bridge Program. For the first time, Medicare is providing a pathway to affordable, dedicated weight-loss medications for eligible beneficiaries. If you are enrolled in Medicare, this pilot program could significantly lower your out-of-pocket costs for popular weight management treatments. Let’s break down the answers to the most common questions about how the program works.

1. Do I qualify for the Medicare GLP-1 Bridge Program?

To enroll, you must be registered in a Medicare Part D plan and meet the criteria for one of three distinct qualification pathways:

  • Pathway 1: A starting Body Mass Index (BMI) of 35 or higher.

    • Note: This includes your BMI before you begin treatment. If you have been paying out-of-pocket and have already lost significant weight, you can still qualify based on your baseline numbers.

  • Pathway 2: A starting BMI of 30 or higher, plus at least one of these conditions:

    • Heart failure

    • Chronic kidney disease

    • Significant high blood pressure requiring 2 or more daily medications

  • Pathway 3: A starting BMI of 27 or higher, plus at least one of these conditions:

    • Prediabetes

    • History of a heart attack or stroke

    • Symptomatic peripheral artery disease (PAD)

 Important Exclusions: You will not be eligible for this specific Bridge Program if you have an existing diagnosis of type 2 diabetes or severe obstructive sleep apnea. If you have either of these diagnoses, your GLP-1 medication should already be evaluated for coverage under your standard Medicare prescription drug benefits rather than this pilot program.

2. Which medications are covered?

The program covers specific brand-name weight-loss drugs:

  • Wegovy® (weekly injection or daily oral pill)

  • Zepbound® (KwikPen® weekly injection formulation only—vials and single-dose pens are excluded)

  • Foundayo® (daily oral pill)

3. What is the out-of-pocket cost?

If you meet the prior authorization requirements, your cost is a flat $50 copay for a 1-month (30-day) supply. Please note that multi-month fills (such as 60-day or 90-day supply scripts) are not permitted or covered under the Bridge Program rules.

4. How long will this program be available?

The Medicare GLP-1 Bridge Program is a temporary demonstration project. Funding and operations are currently scheduled to run through December 31, 2027.
