Lower eyelid blepharoplasty addresses under-eye bags, hollowing, and the shadowed transition between the lower lid and the cheek. The procedure involves either removing the displaced fat or repositioning it into the hollow beneath to create a smooth, continuous contour. The under-eye bags most people describe is not swelling. It is orbital fat that has pushed forward as the membrane holding it back weakens with age, and directly below that bulge the tear trough deepens. The resulting step-off casts a shadow that give the appearance of fatigue regardless of how well rested you are. Two surgical approaches are used: a transconjunctival approach that places the incision on the inside surface of the eyelid and leaves no external scar, and a transcutaneous approach that uses a fine incision just beneath the lash line when significant excess skin must also be addressed.
