What Is a Hair Transplant?

A hair transplant moves hair follicles from the back and sides of the scalp (places where hair is typically resistant to thinning) into areas of loss. Because the transplanted follicles keep the characteristics of the site it came from, the hair continues to grow in its new location for life. Modern transplantation is performed using follicular units, the naturally occurring groupings of one to four hairs in which scalp hair actually grows. Utilizing the natural grouping is what allows a result to be undetectable, and it is the reason most contemporary transplants bear no resemblance to the plugs of decades past. The entire procedure is performed in the office under local anesthesia with oral or intravenous sedation, and typically takes most of a day depending on the number of grafts.

What Are the Different Hair Transplant Techniques?

There is no single best technique, only the technique best suited to a given patient. We offer the full range and select a technique based on your donor supply, hair characteristics, extent of loss, styling preferences, and how much visible downtime you can accommodate.

Linear Strip Excision (FUT)

Also known as follicular unit transplantation (FUT), this method removes a narrow strip of tissue from the donor area at the back of the scalp. The strip is dissected under microscopes into individual follicular units, and the donor site is closed with a trichophytic closure that encourages hair to grow through the resulting line. Strip excision yields the highest number of quality grafts in a single session and does not require shaving the donor area, so surrounding hair conceals the site immediately. It leaves a fine linear scar that is easily covered by hair of moderate length. For patients with substantial loss, and for most women, it remains an excellent choice.

Follicular Unit Extraction (FUE)

FUE removes follicular units individually using a small punch, leaving many tiny circular healing points rather than one line. It is the preferred approach for patients who wear their hair very short, who have limited scalp laxity, or who have had prior strip procedures. FUE distributes the donor harvest across a wider area, which requires careful planning to avoid visible thinning of the donor region itself. In most cases for men, the donor area is shaved. Because each graft is extracted individually, session sizes are typically smaller than with strip excision, but larger than with the unshaven techniques.

Unshaven Hair Transplantation

Unshaven, or modified long-hair FUE allows the donor area to be harvested while leaving the surrounding hair to cover the extraction sites. Patients can generally return to work and social life within days without visible evidence that a procedure was done rather than weeks when the donor is shaved. This approach is especially valuable for women, for patients in visible professions, and for anyone who cannot accommodate an obvious recovery. It is technically more demanding and more time-intensive, which limits session size, but for the right patient the discretion it affords is decisive. Unshaven FUE is the technique used in most cases to remove follicular units for eyebrow and eyelash hair transplantation.

Techniques can also be combined. Using both strip excision and FUE in a single session maximizes the total grafts available, which is often the right strategy for patients with advanced loss.

What Is the Purpose of a Hair Transplant?

The purpose of a hair transplant is to redistribute hair into areas where it has been lost, such as the hairline, the crown, eyebrows, scars and other locations on the scalp, face, and body to add density and rebuild coverage. Good transplantation is as much a matter of design as of technique. For women, follicular grafts can be strategically placed in specific areas of loss to restore a hairline, a side part, or central thinning. For men, a hairline must be planned for the person they will become with age, not only the person sitting in the chair today because hair loss is progressive and donor supply is finite. Angle, direction, and the gradual transition from single-hair grafts at the leading edge to denser multi-hair units behind are what make a result look natural. The goal is a natural, durable outcome that continues to look appropriate over time. To maintain density, it is imperative to be on medical or non-medical therapies after a hair transplant to slow down progressive loss of non-transplanted hair.

What Are the Benefits and Risks of a Hair Transplant?

Hair transplantation is the only treatment that reliably restores hair to an area where follicles have been lost. When performed by a trained surgeon with an experienced team, it delivers permanent, natural-looking results. As with any surgical procedure, understanding the benefits and potential risks helps ensure you make a confident and informed decision.

Benefits

  • Permanent restoration using your own hair, which grows, can be cut, and can be styled normally.

  • Natural results when follicular units are placed at correct angle, direction, and density.

  • Restores the hairline and adds density where medical therapy alone cannot.

  • A range of techniques, including unshaven options with minimal visible downtime.

  • Performed under local anesthesia in the office, with most patients back to routine activity within days.

  • Effective for eyebrows, beard, and scarred areas as well as the scalp.

Risks

  • Swelling of the forehead and around the eyes during the first several days.

  • Numbness or tingling of the scalp, usually resolving over weeks to months.

  • Shock loss, a temporary shedding of existing hair in the donor area which recovers.

  • Folliculitis or ingrown hairs as new growth emerges.

  • A fine linear scar with strip excision, or small dot scars with FUE.

  • Variable graft survival, with results depending heavily on graft handling technique and post-operative care.

  • Continued loss of non-transplanted native hair, which is why ongoing medical therapy matters.

  • A second session may be needed to achieve the density desired.

  • Rare complications such as infection or poor blood supply, underscoring the importance of choosing a skilled and trained hair restoration surgeon.

What Should You Expect After a Hair Transplant?

Recovery follows a predictable pattern that is worth understanding in advance, particularly the shedding phase, which can be alarming.

  • Days 1 to 3: the most important time to prevent facial swelling and optimize follicular graft survival by following our post-operative regimen.

  • Days 5 to 10: the grafts become more secure and small crusts around each graft begin to fall away.

  • Weeks 2 to 4: some transplanted hairs shed. This is expected and does not mean the grafts have failed; the follicle bulb remains in place and has entering a resting phase.

  • Months 5 to 6: new growth begins to appear.

  • Months 6 to 9: most of the growth becomes visible.

  • Months 12 to 18: the final result, with hair reaching full caliber and density.

Patience is genuinely part of the process. The result at six months is not the result you will have at a year.

How Can Well by Messer Help You with Hair Transplant?

At Well by Messer, hair transplantation is performed by a surgeon fellowship-trained in both hair restoration surgery and oculoplastic surgery who works across the full range of restoration techniques rather than specializing in only one. That matters, because a practice that offers a single method will tend to recommend that method to everyone. Our planning takes a long view: we assess donor supply carefully, design conservatively, and combine surgery with medical therapy to protect the native hair still present, because a transplant that is not paired with medical treatment can leave a restored hairline sitting in front of continued thinning behind it. We treat all hair types and textures.
