What Is an Eyebrow Lift?
An eyebrow lift, also called a brow lift, raises the eyebrows to the desired height and shape. As the brow descends with age, it pushes skin downward onto the upper eyelid, creating heaviness and crowding around the eye that eyelid surgery alone cannot correct. Several surgical approaches exist, and the right one depends on brow position, forehead height, hairline, and skin quality.
Internal Browpexy
Performed through an existing upper eyelid incision to stabilize and modestly elevate the outer brow, most often at the time of blepharoplasty.
Temporal or Lateral Brow Lift
A limited elevation of the outer third of the brow, where descent is often most visible. The scar is hidden within the eyebrow hairs. Any further camouflage of the incision can be achieved with hair follicle grafts or micropigmentation after six months.
Pretrichial (Hairline) Brow Lift
An incision placed at the frontal hairline that lifts the brow without raising the hairline. A specialized closure allows hair to grow through the scar line, and hair follicle grafts can further camouflage the incision at any point after six months.
Direct Brow Lift
A precise excision immediately above the brow hair, reserved for patients with significant asymmetry or facial nerve weakness (Bell’s palsy).
What Is the Purpose of an Eyebrow Lift?
The purpose of an eyebrow lift is to restore the brow to its proper position so that the upper face appears open and relaxed rather than tired or stern. Brow descent is frequently mistaken for excess eyelid skin, and the distinction determines whether surgery succeeds. A simple test is to gently lift your brows with your fingertips: if the heaviness in your upper lids largely resolves, the brow is a significant part of the problem. Performing eyelid surgery in that situation can pull the brow lower still and worsen the very appearance you were trying to correct. Brow lifting also addresses lateral brow descent, where the outer third of the brow falls below the orbital rim and contributes disproportionately to a heavy, hooded look.

Over-elevation producing a surprised appearance if the lift is too aggressive.
