What Is Hypothyroidism?

Hypothyroidism is a medical condition characterized by an underproduction of thyroid hormones by the thyroid gland. In hypothyroidism, the thyroid gland is unable to produce adequate amounts of the thyroid hormones, thyroxine (T4) and triiodothyronine (T3). Hypothyroidism can be mild (subclinical) or more significant (overt). This hormonal deficiency results in a generalized slowing of metabolic processes. Biochemically, hypothyroidism is typically indicated by elevated serum thyroid-stimulating hormone (TSH) levels, alongside decreased serum concentrations of T4 and/or T3. The reduced thyroid hormone levels affect various bodily systems, leading to decreased basal metabolic rate, impaired thermoregulation, and altered cardiovascular function.

What Are the Causes of Hypothyroidism?

The most common causes of hypothyroidism include:

  • Autoimmune disorders, such as Hashimoto's thyroiditis

  • Surgical removal or radiation treatment of the thyroid gland

  • Certain medications, such as lithium or amiodarone

  • Iodine deficiency (rare in the United States)

  • Congenital hypothyroidism (present from birth)

The symptoms of hypothyroidism can include*:

  • Fatigue and sluggishness

  • Weight gain or difficulty losing weight

  • Cold intolerance and feeling cold

  • Dry skin and hair, and brittle nails

  • Constipation

  • Depression and cognitive impairment

  • Menstrual irregularities in women

* If due to hypothyroidism, these symptoms should resolve with treatment.

Hypothyroidism is diagnosed through blood tests that measure the levels of thyroid hormones (T4 and T3) and thyroid-stimulating hormone (TSH).

What Are the Treatments and Therapies for Hypothyroidism?

The primary treatment for hypothyroidism is the replacement of the missing thyroid hormones through daily oral medication. This can be done through synthetic or “natural” thyroid hormones. The standard treatment is levothyroxine, a synthetic form of thyroxine (T4), administered orally. Dosage is tailored to the individual based on factors such as age, weight, severity of hypothyroidism, and presence of comorbid conditions. Regular monitoring of serum thyroid-stimulating hormone (TSH) and free T4 levels is essential to adjust dosage and ensure therapeutic efficacy. In some cases, liothyronine (synthetic T3) may be used, either alone or in combination with levothyroxine, though this is less common. The goal of treatment is to restore normal thyroid hormone levels and alleviate the symptoms of hypothyroidism.
